Alex has psychological issues with being male. I think its a bit quick to judge, but one of the most important aspects of reidentifying as the correct sex is to acknowledge and accept the physical form. You can't pretend that this unique experience didn't occur or is untrue.
My basis is that he lifts his tail after being told he's pretty (rather than handsome?). Further, he immediately finds it important to hide his balls rather than act in any sensible manner.
